Palliative RN Case Manager

This is a Full-time position in Las Vegas, NV posted November 25, 2022.

The Palliative Registered Nurse provides outstanding care to patients with Advanced Illnesses in their home environment. This care is based upon the nursing process; assessment, nursing diagnosis, planning, implementation and evaluation. They must also provide excellent communication and coordination of care with patient s primary care provider, other Palliative Team members and miscellaneous community providers such as specialists and home health. Stellar clinical documentation is required. The Registered Nurse oversees and guides employees under his/her supervision including LPNs and CNAs.

Shift: Monday-Friday 8am-5pm

The Palliative RN Case Manager responsibilities include:
Participate in interdisciplinary team meetings
Telephone access to clinical professional for patients
Education for the patient and families about living with advanced illness
Advance Care Planning
Assistance with navigation and referrals to community resources for needs related to illness
Access assistance to professional support such as specialist consultation, home health, and medical equipment, spiritual and volunteer when necessary.
Tracking admissions to the hospital and coordinating palliative support upon return home
Assessment, creation and maintenance of plan of care
Implement FACIT PAL surveys for each new admission
Nurse visits to the home as determined by the Interdisciplinary Team
Patients and families will receive assistance with medication management oversight, ensuring proper nutrition, home safety evaluation, and access to care or guidance around the clock. This will enable patients and families to have a sense of normalcy in their lives allowing them to participate in social activities, remain independent and enjoy quality of life.
Communicate updates post visit and coordinate all patient care with patient s Primary Care Physician
Coordinate consultation appointment at the palliative clinic

Education and Experience:
Associates Degree in Nursing; BSN strongly preferred
Hospice and/or home health experience preferred
Must have competency on required technologies such as electronic medical records, smart phone/PDAs, etc.
Required Licensure and Certification:
Registered nurse with active, unrestricted Nevada RN license or ability to achieve prior to hire.
Current CPR certification.
Must also be a licensed driver and possess an automobile that is insured in accordance with state and/or organizational requirements and is in good working order. Must maintain a clean driving record.
